Hillsdale Hospital is pleased to announce the arrival of over 1,000 Pfizer vaccinations for high school students ages 16 and above. If you have a student in a Hillsdale County Public School within that age range, vaccination clinics are taking place this week! This is a voluntary program and will require students to have a consent form signed by a parent or guardian. Hillsdale Hospital clinical staff will be traveling to the districts to administer these vaccinations, and your student can participate with your approval.
Below you will find the registration links for each vaccination event. Before you go any further, please read the following:
A parent/guardian will be required to register their student and confirm on the registration that their student is 16 years of age or older. Parents, please include your email and phone number, not your student’s, when completing the registration. Also, understand that it will be a first-come, first-served basis throughout the established time period for each clinic. We are only using this system for registration and student counting, not for a set vaccination time. Contained in the main event details is our parental consent form and vaccination consent form (indicated for Pfizer). The lack of a completed Hillsdale Hospital consent form will result in no vaccination administered.
